I.-The accreditation procedure is regional. The list of local hospitals is drawn up for each region by the Director General of the Regional Health Agency.

The establishment or site applying for inclusion on the list of local hospitals sends its application to the Regional Health Agency by any means that provides a date certain of receipt.

This application enables the Director General of the Regional Health Agency to assess whether the establishment or site meets the eligibility conditions for inclusion on the list of local hospitals referred to in II of article R. 6111-24. The Director General of the Regional Health Agency shall take into account the elements of the regional health project, the characteristics of the healthcare provision in the region and the project and prospects to which the establishment or site is committed on a long-term basis in order to meet the health needs of the population.

The content of the application file shall be determined by order of the Minister for Health.

II.-If the application of an establishment or site is rejected, the Director General of the Regional Health Agency shall send his decision, stating the reasons, by any means that ensures certainty of receipt. The identified establishment or site may not reapply for a period of one year.

III.-Establishments or identified sites included on the list may only be removed from it under the following conditions:

a) If the establishment concerned informs the Regional Health Agency that it no longer wishes to be included on the list;

b) If the establishment no longer meets the conditions mentioned in 2° or 3° of II of Article R. 6111-24;

c) If the establishment no longer permanently meets the conditions mentioned in 1°, 4° or 5° of II of article R. 6111-24;

d) If particular circumstances make it impossible for the establishment to ensure continuity of care for patients, without prejudice to any measures that may be taken to guarantee patient safety and the quality of their care.

The establishment is removed from the list of local hospitals in the region after a period set by the Director General of the Regional Health Agency, which may not exceed twelve months in the cases provided for in a, b and c and six months in the case provided for in d.
